Final Step & Submission

The following documentation is required in order to open your new account.  Please print and complete each form and submit according to the guidelines below.

  1. Download DocumentContract Specifications:  Sign to accept all disclosures as stated and agree to the terms of trading as well as to confirm the accuracy of all information provided herein.
  2. Download DocumentW-9: US Citizens are required to complete all sections of the form as a declaration of personal identity and Social Security number. or Download DocumentW-8(BEN):  Non-US Citizens are required to complete all sections of this form in order to declare country of residency as well as tax treaty status.
  3. Identification:  Please submit an unexpired photo ID such as Passport, Driver’s License, or other government-issued identification card.
  4. Proof of Address:  Please submit a utility bill, bank statement, brokerage statement or other document dated within the last 90 days to verify the address listed on your application.

If Applicable:

  1.  Download DocumentAdditional Risk Disclosure:  Complete the Additional Risk Disclosure only if the account holder declares a net worth or an annual income of less then $25,000 or if the account holder is under 21 or over 75 years of age.
  2.  Download DocumentLimited Power of Attorney:  Complete the LPOA only if a party other than the account holder will be trading the account.  Please complete the LPOA including the statement of any fees agreed upon by the concerned parties.
